How do you know if it’s hemorrhoids or something else?

It’s just on the outside where you wipe and hurts. But if it comes and goes about every three months it’s just hemorrhoids, right?

Yeah you could be right. It is may be an external hemorrhoids.
There are Symptoms of hemorrhoids include:
•rectal bleeding
•pain around the anus and rectum
•irritation and itching
•bulge or lump at the anus
Excessive straining, rubbing or cleaning the anus may worsen symptoms. In most cases, the symptoms of hemorrhoids will only last a few days. Not everyone with hemorrhoids will experience symptoms.
You can try some hemorrhoids treatment to avoid it getting worse, like Controlling constipation and diarrhea, consuming a lot of water drink (8-10 glass/day), eat some fruit, etc
